It keeps the gun/holster from digging into your pelvis during appendix carry, and also aids in concealment by putting opposite pressure to keep the top of the gun pressed into your body. Also helps with hot spot reduction. Once I tried the wedge I'll never not use one again with appendix. I attached the hook and loop to both so it's removable for other carry positions or pocket/bag carry.
